Our team works closely with District Nurses and NHS palliative care specialists to make sure every aspect of care is thoughtfully managed in the comfort and familiarity of your own home.

Medication assistance

Our carers are trained to ensure your medications are managed safely and reliably, liaising with GPs to ensure smooth transition should your medication needs change.

Home support

From mobility support to meal preparation and daily chores, our carers can do whatever is needed to look after you and your home.

Personal care

Our compassionate and professional carers can help with daily routines like washing, skincare and oral health as well as assistance with dressing and getting prepared for the day.

What is palliative care?

Palliative care is a term used to describe the care provided to improve quality of life for a person facing a life-limiting illness. Palliative care also involves psychological, social and spiritual support, taking full consideration of your thoughts and feelings surrounding diagnosis. When the time is right, palliative care dovetails into end-of-life care, and we work closely with District Nursing teams to support pain management, to ensure the death of a loved one can be as dignified and peaceful as possible in the comfort of their own home.

How can Flexicare’s palliative team support my loved one?

We know that this is a most difficult time for you and your family – and that having a comprehensive care package planned around what’s most important to you is essential for your wellbeing. Flexicare can ensure a seamless transition from a domiciliary care package to the introduction of the palliative team – which can work in tandem with your usual team of carers to ensure you always feel connected to your care and support. By working closely with palliative nurses, GP Clinics and District Nursing teams, we support a holistic approach to your care provision, ensuring you always remain at the centre of your care plan. Flexicare is also a provider of CHC fast track funding care – funding that anyone who is nearing end-of-life is eligible for. This shows that both local authorities and the families we currently care for place their trust in Flexicare to provide compassionate, reliable care when it matters most.

What funding is available to assist with palliative care?

CHC funding is available if you are eligible. This is assessed by the District Nursing team who are able to put through a fast-track funding which is normally available within 48 hours. You have full autonomy of choice when it comes to selecting your care provider. If you are already a client of Flexicare, the transition between your regular team of carers and the enhanced palliative team will be seamless. If you are new to Flexicare, our enhanced palliative team lead will attend your home to discuss the package of care and reassure you on how the care provision will look and fit into your family. They can also discuss timings that may be more suitable and adapt if needed, as well as offering emotional support to your loved one and the rest of the family.

Which multi-disciplinary teams does Flexicare work with?

Flexicare will work very closely with District Nursing teams, GP clinics, palliative nurses and also with GIS for equipment delivery.
